What we collect
When you fill out a form on this website, we collect the information you choose to give us, such as your name, phone number, email, city or zip code, practice or firm name, facility address and your message. If you request our free Accident Recovery Guide, we collect your first name, mobile number and, if you choose, your email. We ask that you not share detailed medical information through the website. We'll go over that with you by phone.
How we use it

- To connect you with the providers, attorneys or partners you asked us to help you reach
- To respond to questions about joining our network or working with Cruize
If you check the consent box on a form, you agree that we may contact you by phone or text about your request. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help.
What we don't do
We do not sell your personal information. We only share what you give us with the people needed to help with your request, and only for that purpose.
How it's stored
Form submissions are processed by our website host and delivered to the Cruize Control Healthcare team. We use reasonable safeguards to protect them.
